1K - $42K a year HRCSB is seeking a full-time Care Coordinator Specialist to join our agency in assist with facilitating care for individuals and families engaged in mental health and/or substance use outpatient and medical services.
Looking for an individual who is passionate about client care and interested in working in a team environment to support clients.
The Care Coordinator will work closely with clients and their providers to ensure effective communication across providers, assist with any referrals to additional support services or medical services in the community and seek client feedback on their services.
Strong communication skills and ability to work with clients directly is essential.
Qualified candidates must have at least one year working with individuals with mental health and/or substance use issues or experience working in the medical field.
